Attendance Verification Forms

Students who are going to test for their driver's license or permit need to obtain a student enrollment verification form from the attendance office. Students will be asked to fill out the form and return it to Mrs. Hanna to be sent in to the school board. The process may take awhile, so having the form sent in a week before the date of the test is recommended. However, signed and stamped forms have a 30 day expiration date.
